What changes inside the MP4

An MP4 is a container. What makes it large is the video stream inside, so compressing an MP4 means re-encoding that stream at a lower bitrate. The container, the file extension and the compatibility stay the same: the output is an MP4 with H.264 video and AAC audio.

Which level should you pick?

Smaller file scales the long side to about 854 pixels, Balanced to about 1280, and Best quality keeps the original size. If a level would make the file larger, the original is returned unchanged.
